 I receive a steady stream of applications for a first appointment with me. While most of these are sincere requests for my time, some, unfortunately, are not. I need a way to separate serious applicants from frivolous ones, a method which is fair both to those serious applicants and to me. Would you therefore be willing to make a small advance deposit on your very first appointment with me? I will give a higher priority to first-time applicants who make such a small gesture of their sincerity and seriousness than to those who do not.

I prefer office numbers that can be called between 11:30am-5:00pm Eastern time. The larger your time frame, the easier it is for me to call you. Exact times are hard for me to accommodate. But will try. Any message left will be discreet.

No cell phones or 800 numbers for initial calls. You may include cell phone if office number is also provided. If you have a hotel phone, please mention that in the "Additional Comments." I can sometimes call hotel phones after 7:00pm but prefer not to make home calls past 6:00pm.

My preferred range is a time BLOCK from 12 to 5pm. Calls before 10:30am or after 6:30pm are difficult for me to accommodate also exact times are hard to accommodate.

If you have any extenuating circumstances associated with your application that you feel need explaining, please note here. For instance some of my physician applicants can only be called at specific times or other applicants wish me to identify myself in a special way. Take note, I will respond to you with discretion. Communication with you will be discreet and will focus on the logistics connected with setting up your first appointment. I will refrain from indiscreet details in my communication to you. Unless you have a medical limitation problem that needs to be mentioned, physical descriptions about your appearance need not be mentioned.
